How do I make changes to my account information?
- Add or remove a person on the account: The primary account holder must complete a new service request form and provide a copy of their driver’s license.
- Update last name due to marital status: The primary account holder must complete the form and submit a copy of their updated driver’s license and copy of marriage certificate.
- Change name due to other legal name change: The primary account holder must complete the form and provide court documents acknowledging the change and provide a copy of their driver’s license.
- Change to mailing address: Contact our office to obtain a change request form. The primary account holder will need to complete and submit the form with a copy of their drivers license.
