– Health inspections

A Health Inspection is an inspection required by the County for all retail food establishments.

To complete the Water District Approval form, MOC will conduct a CSI Inspection and provide the signed form stating the MUD provides water and sewer to the location.

To schedule this inspection with MOC, please email the following to commercialservices@municipalops.com:

1. A Water District Approval Form – For your convenience, the forms for several counties are linked below. Please fill out the form with the name of the business, service address, and MUD, and leave the rest of the form blank.
2. Current Backflow Test Certificate – Backflow is a plumbing mechanism that permits water to flow into a building while preventing reverse flow, thereby safeguarding the public water supply from contamination. It must be inspected annually by a certified inspector, who issues a test certificate to confirm its proper operation. A certificate for each meter at the location is required; the property owner or landlord should provide this to you.
3. Contact information: Please include the name and phone number of the contact person for the inspection.

Once this information has been received by MOC, we can proceed with scheduling the Health Inspection.
